P.A.T.H. — Partners for Assistance, Transformation, and Hope — offers shared housing for adults who can safely manage their daily needs independently. We aim to provide a welcoming home environment that encourages personal responsibility, respect for others, and community connection.

Shared accommodations
Furnished sleeping accommodations in a shared residential setting, with access to common household spaces.
Everyday essentials
Utilities, Wi-Fi, shared appliances, basic household supplies, and routine household upkeep.
Respectful environment
A drug-free, smoke-free home with clear house expectations, quiet hours, and consideration for housemates.
Meals, groceries, transportation, medical care, personal care services, and other individualized services are not included in standard room and board unless separately arranged.
Independent living with shared community.
We welcome inquiries for adult men, including seniors, veterans, and adults with disabilities, when the shared living environment is an appropriate fit.
General eligibility
- Age 18 or older
- Able to independently manage daily living needs
- Able to safely navigate stairs and evacuate the residence
- Medically stable and appropriate for shared housing
- Willing to follow house rules and respect others
Each referral is reviewed individually for suitability and available space.
Referrals are welcome.
We welcome inquiries from hospital discharge planners, social workers, care coordinators, guardians, community agencies, and families. Please contact us to discuss eligibility, availability, and next steps. Do not include diagnoses, medication lists, Social Security numbers, or other sensitive personal information in an initial email.

P.A.T.H. provides shared independent housing. It is not a licensed assisted living facility, skilled nursing facility, or provider of 24-hour personal supervision. Any outside care services must be separately arranged with appropriately qualified providers.
